Rationale, aims and objectives Given the increasing prevalence of cataract and demand for cataract extraction surgery, patients must often wait to undergo this procedure. We validated a previously developed priority scoring system in terms of clinical variables, pre-intervention health status, appropriateness of surgery and gain in visual acuity (VA) and health-related quality of life (HRQoL).Methods Explicit prioritization criteria for cataract extraction created by a variation of the Research and Development (RAND) and University of California Los Angeles appropriateness methodology were retrospectively (...) applied to a prospective cohort of 5257 patients on waiting lists to undergo cataract by phacoemulsification at 17 hospitals in Spain. Demographic data, clinical data and data related to surgical technique were collected by trained ophthalmologists. Patients were evaluated by their ophthalmologist before the intervention and 6 weeks afterward. They also completed, by mail, the Visual Function Index (VF-14) before the intervention and 3 months afterward.Results High-priority patients experienced greater improvement in VA and HRQoL than those classified as intermediate or low-priority (P < 0.0001), even after adjusting by VA and the VF-14 score at baseline. The time to intervention was the same for high-priority patients as it was for intermediate and low-priority patients.Conclusions The priority score we developed identified patients most likely to experience the greatest improvements from cataract extraction. Use of this tool could provide a fairer and more rational way to prioritize patients for cataract extraction. (shrink)

xxiii + 293. Price £50.00 h/b). Thinking About Knowing. By JAY F. ROSENBERG. (Oxford UP, 2002. Pp. viii + 257. Price £30.00 h/b). Epistemology is currently enjoying a renaissance. To a large extent, this has been sparked by some exciting new proposals, such as the contextualist theories advanced by Stewart Cohen, Keith DeRose, David Lewis and Michael Williams, the modal conceptions of knowledge offered by Fred Dretske and Robert Nozick, and the virtue epistemologies put forward by John Greco, Ernest Sosa (...) and Linda Zagzebski, to name but three currently popular views. Increasingly, however, this rebirth in epistemological theorising has been driven less by the production of new theories and more by the application of the latest batch of novel proposals to other areas of philosophy. A good illustration of this from the selection of books under review here is the contemporary debate regarding the vexed relationship between content externalism and self-knowledge that is the 2 focus of the volume of essays edited by Susana Nuccetelli. Here we have a controversy that has blended some of the most innovative aspects of recent epistemological theorising with issues in the philosophy of mind and language, with the emphasis being on the so-called ‘McKinsey paradox’ concerning the putative incompatibility of content externalism and privileged self-knowledge. Whilst early responses to this supposed paradox concentrated on the formulation of the content externalist thesis whilst treating the epistemic concepts at issue as, essentially, primitives, more recent work in this area has drawn-out some of the epistemological implications of this debate and looked at how these implications fit in with recent movements in epistemology. Many of the articles collected in this volume are instances of this ‘second phase’ of the debate. In particular, it is tremendously useful to have the new articles by Martin Davies and Crispin Wright which revisit a previous exchange between the pair where some of the epistemological morals of the McKinsey debate were first extracted.. By the death, last summer, of Jack Robson, the world of utilitarian studies and a wider world of scholarship on both sides of the Atlantic lost one of their most distinguished figures. It would not be appropriate here, even if it were possible now, to attempt a full and measured assessment of his work. Writing only a few months after the news of his death, while the sense of loss is still so sharp for all his many friends, two things (...) are possible. Something can and should be said to acknowledge and celebrate Robson's achievement as a scholar; and to this can be added some personal recollections of one whose human qualities were as outstanding as his scholarship. Selected response items and constructed response items are often found in the same test. Conventional psychometric models for these two types of items typically focus on using the scores for correctness of the responses. Recent research suggests, however, that more information may be available from the CR items than just scores for correctness. In this study, we describe an approach in which a statistical topic model along with a diagnostic classification model was applied to a mixed item format formative test (...) of English and Language Arts. The DCM was used to estimate students’ mastery status of reading skills. These mastery statuses were then included in a topic model as covariates to predict students’ use of each of the latent topics in their written answers to a CR item. This approach enabled investigation of the effects of mastery status of reading skills on writing patterns. Results indicated that one of the skills, Integration of Knowledge and Ideas, helped detect and explain students’ writing patterns with respect to students’ use of individual topics. (shrink)

The Peanese convention for the use of dots as brackets has the disadvantage that it gives only an awkward method for representing chains of indefinite length, such as the compound implicationSuch chains occur frequently in logical investigations of a metatheoretic nature, and it is convenient to have a systematic method of abbreviating them. The most obvious method of doing this would be to leave the parentheses out entirely, and to understand that in such cases the implication sign or other operation (...) appearing on the extreme left is the most inclusive; but this method, which has been followed by Quine, is at variance with our ordinary algebraic usage, in which we write a − b + c − d for ((a − b) + c) − d, and consequently leads to confusion. It is desirable to have a modification of the Peanese convention which gives a simple method of representing such chain implications, and at the same time avoids this difficulty.Such a modification is easily obtained by generalizing somewhat a procedure of Church. The latter author uses only a single dot, which he writes on the right of an operator to signify a bracket extending from that point to the end of the formula (or parenthesized expression). If dots are used only on the right of operations this is all that can be desired. But the essential idea of this device can be extended to the case where dots are used also on the left, as follows: let us suppose that a group of dots on the right of an operation or prefix denotes the beginning of a bracket which extends to the right until it meets a group with an equal or larger number of dots on the left of an operation; and that the scope of a group of dots on the left of an operation shall extend to the left until it reaches a larger group of dots on the right of some operation. The Greenberger-Horne-Zeilinger state is the most famous example of a state with multiparticle entanglement. In this article we describe a group theoretic framework we have been developing for understanding the entanglement in general states of two or more quantum particles. As far as entanglement is concerned, two states of n spin-1/2 particles are equivalent if they are on the same orbit of the group of local rotations (U(2)n). We consider both pure and mixed states and calculate the number of independent (...) parameters needed to describe such states up to this equivalence. We describe how the entanglement of states in a given equivalence class may be characterized by the stability group of the action of the group of local rotations on any of the states in the class. We also show how to calculate invariants under the group of local actions for both pure and mixed states. In the case of mixed states we are able to explicitly exhibit sets of invariants which allow one to determine whether two generic mixed states are equivalent up to local unitary transformations. Empirical and theoretical evidence show that the astrophysical problem of dark matter might be solved by a theory of Einstein-Mayer type. In this theory, up to global Lorentz rotations, the reference system is determined by the motion of cosmic matter. Thus, one is led to a “Riemannian space with teleparallelism” realizing a geometric version of the Mach-Einstein doctrine. The field equations of this gravitational theory contain hidden matter terms, where the existence of hidden matter is inferred solely from its gravitational (...) effects. It is argued that, in the nonrelativistic mechanical approximation, they provide an inertia-free mechanics, where the inertial mass of a body is induced by the gravitational action of the comic masses. Interpreted from the Newtonian point of view, this mechanics shows that the effective gravitational mass of astrophysical objects depends on r such that one expects the existence of dark matter.
